Quality Improvement and Population Health

Using the data and sophisticated health information technology available at NorthShore, our residency curriculum fully integrates population health into patient care. Highlights of our curriculum include:

  • Training in the innovative My Panel dashboard tool, and advanced disease registry that allows residents to manage their population's chronic disease and preventive care through our EHR
  • Collaboration with on-site case management team to improve the care of complex patients
  • Longitudinal quality improvement curriculum
  • Opportunities for team and individual quality improvement projects
  • Teamwork / leadership training as part of one of our three patient care teams in the Family Care Center
  • Advocacy opportunities through the Illinois Academy of Family Physicians and participation in Spring Into Action
